Please review the information below. If you wish to enroll your student for the Fall 2017 sign and return this document to Tune In Music Academy before August 14th along with along with applicable fees.
The Fall semester August – December is 16 weeks long.  Lessons normally correspond with the public school academic schedule.  Students expected to be present for the lessons every week.  Lessons are by appointment only. Time slots are scheduled prior to the start of each school year.  Semester starts on Tuesday, August 22nd and ends on Friday, December 16th.  There will be Monday, 20th November - Sunday, November 26th off on Thanksgiving week.  All other days are lessons days.
Tuition and Payment

Lessons rates are $33 for 30 min, $48 for 45 min, and $65 for an hour. Payment includes 16 individual weekly lessons and registration fees.  Payment is due by August 14th.  Early payment will reserve your spot in our studioTuition may be paid in full amount or by installment plan, with monthly post-dated checks that are provided per semester. Payments are accepted by Checks or cash only.  Tuition by check must be payable to Tune In Music Academy.  Payments can be drop off in the studio or mailed to:

There is a $20 fee for late or returned payments.

To pay by installment plan, turn in 4 checks by August 14th for the installment amount dated:
8/14/17, 09/14/17, 10/14/17, 11/14/17
Make-up policy
Students are allowed to reschedule their individual lesson ONLY two times per semester which should be taken within 2 weeks.  Notify the teacher at least 24 hours in advance if the student cannot attend the lesson.  There are no refunds for missed lesson.  Occasionally the teacher might have schedule conflict due to performing activity.  If a teacher is absent, a make-up lesson will be scheduled at a mutually convenient time. Scheduled make-up lessons missed by the student will not be made up. Fall semester lessons will not be transferred into the next semester and has to be completed before December 16th.
If you decide to discontinue lessons in the middle of the semester you will be giving the refund for remaining lessons.  The registration and late fees are non-refundable.  Any termination initiated by the parent or student requires 1 month WRITTEN notice.  The teacher may terminate lessons due to negative behavior, failure to practice, or failure to comply with studio etiquette. 
Indemnification, Waiver and Publicity Release Form
Students must have Indemnification, Waiver and Publicity Release Form on file with Tune In Music Academy in order to take lessons.
